UCLP risk scores range from 1-5 with 1 meaning high risk and 5 meaning low risk. You can sort your patients by this risk score.

This score was developed by University College London Partners (UCLP), one of the world's leading academic health science partnerships. UCLP combines innovative research and clinical practices to develop various risk stratification tools. These tools are designed to identify patients at high risk of certain conditions or those who may benefit from specific interventions, thus enabling more targeted and efficient healthcare delivery.
They analyse various patient data (such as medical history, demographic information, and clinical outcomes) to categorise patients according to their risk of developing a particular condition or their need for certain medical interventions. This approach enables more personalised care plans and can lead to improved health outcomes.
Why do we use UCLP Scores?
By identifying at-risk patients early on, we can intervene earlier in a patient’s journey, reducing the likelihood of severe outcomes and improving the overall quality of life for patients. For example, early identification of patients at risk of chronic diseases such as diabetes or cardiovascular diseases can lead to interventions that significantly reduce disease progression and complications.
UCLP Risk Scores are used at Suvera to classify the priority of patients invited to use our service alongside any other configuration requirements we’ve agreed with your practice.
