How do I recall patients for questionnaires?
Patients can submit ACT, smoking status, BMI, and BP diaries with Suvera questionnaires

Suvera is multi-morbidity by nature. Our questionnaires put everything a patient needs behind 1 link which lasts a full 14 days.


That means if they need to submit their ACT, smoking status, BMI, and BP diaries, it can be done at once to minimise SMS spam.


Here's how to get started:

1. Click "Recall" on a patient to see which questionnaires they'll be recalled for. Click "Edit" if you want to change this or see more details.


2. Click "Questionnaires" to add or remove a questionnaire from the recall. Click "Preview" to see what questions the patient will be asked. These are all QOF-required questions and will be QOF-coded as monitoring invites in your EHR. Click "Continue" when you're happy.


3. The patient will receive a link to submit their answers.


4. You'll see all their answers in the "Inbox" section of Planner. Patient submissions are colour coded red, orange and green depending on how out of control their ACT scores or BP readings are. You can save these submissions to your EHR whenever you're ready.
