If you use Suvera's Virtual Clinic, you can refer a patient to Suvera through Planner at any time and for multiple different reasons:

• to highlight a patient for Suvera to review
• to have Suvera pick up any follow-up care for a patient you have reviewed
• to prioritise a patient and their care.

Step 1

You can make a referral in two different ways, which are:

• Through the Create a Referral tab in the left-hand side menu
  
• Through the patient's page by clicking Options and then Create a Referral
  

Step 2

In order to submit a referral, all you’ll need to do is fill in the patient’s NHS number and let us know what it is you’d like us to action.

Step 3

You’ll then be asked to confirm this patient meets our eligibility criteria, at which point your referral will be complete.

Step 4

You can track the status of all referrals you have sent to us via the Past Referrals tab in the left-hand side menu, where you can also filter them by status.
